The interdisciplinary methodology to individualize care in each resident is based on work for specific

objectives in the different areas of personal autonomy (ADL, Mobility, Cognitive Functions and

Relationship / Integration) that is developed in the following phases of intervention:

1. Initial interdisciplinary evaluation:

The physical, mental and social situation of the new admission will be evaluated during the first days

of stay by the different members of the Team.

2. Initial interdisciplinary

assessment : design of the individual plan

- Prioritize needs: a team meeting will be held to, with a pooling of the information collected, assess the clinical, functional, cognitive and social status of the resident, identify –prioritizing- their needs, and design the individual intervention plan.

- Agree and define the objective: Related to the following four areas of autonomy: ADL, mobility, cognitive functions and social relationship.

- Include in specific intervention programs:Depending on the objectives, we will proceed to refer to the specific programs that are considered appropriate.

- Prescribe specific guidelines: Specific guidelines (medical, social, environmental, behavioral, etc.) will be prescribed in the different programs for each case.

- Exchange and consensus of the intervention: It will be carried out by agreeing with the resident and

/ or the family on the individual plan and their participation in it. In this way, the individual plan

formalized in the corresponding document (Comprehensive Intervention Protocol) is constituted as a care contract between the center and the person attended.

3. Intervention and monitoring of the Individual Plan: tutoring system.

The different professionals of the center will apply the individualized plans following the actions

and guidelines agreed interdisciplinary for each case. To facilitate said application and monitoring, each center will proceed to organize the tutoring system: proportional allocation to residents' monitoring aides Each assistant / tutor - or person of reference - will carry out continuous monitoring of the assigned cases, consisting of assessing their evolution (bimonthly) according to the evaluation protocol by areas of autonomy, and will participate in the team meetings established for said cases. This monitoring system is complemented by the collection of information in the common

instrument - incident sheet - on events or incidents that, due to their unusual nature, may alter the individual plan.

4. Evaluation of the individual plan

The general evaluation of the Individual Plan, by the entire Team, will be carried out every six months. In it, the individual situation will be evaluated again (new assessments, new needs, changes in the individual intervention plan, change of guidelines). However, for different reasons (unexpected

situations, inadequate operation of the intervention plan, difficulties in applying the guidelines, etc.), the assistant tutor or reference person may request follow-up meetings before the six-month period.
